Our Programs

Duffy Health Center provides integrated services to those in our care so that they can overcome barriers, restore their health and rebuild their lives. Our programs are tailored to meet the unique needs of the population we serve.

Housing First

Duffy Health Center was the first Cape-based organization to adapt this national model. Housing First provides stable housing as a first step in services for homeless individuals. By eliminating the chaos of homelessness, Housing First participants are able to more effectively address underlying behavioral health and medical issues. Housing First is an evidence-based practice and has been a factor in national and state funding shifts away from emergency shelters and towards permanent, supportive housing models. At Duffy, Case Management staff work with local public and private housing authorities as well as many private landlords who value the case management support that help people achieve and maintain successful tenancies. Housing First has resulted in a decrease in risk behaviors and utilization of public services and increased positive connections to preventive health services and to the community.

In from the Streets

Duffy Health Center manages this community-wide program, which since 2008 has offered temporary motel shelter and case management services for individuals experiencing homelessness who are medically medical frail or at risk due to severe weather. Duffy’s integrated care model, skilled case managers, and timely response keep vulnerable people held within the safety net of care. Our close collaboration with community partners – human service providers, police departments and the faith community – reduces duplication of services. Behavioral Health Community Partners

Patients with the most significant behavioral health needs receive specialized support from Duffy Health Center case managers as they navigate the health care system. Support offered by case managers includes care coordination and care management; support of transitions of care; and connection to social services and community resources so that patients can live independently.

Bridge Program

With funding from our federal Health Care for the Homeless grant, Duffy partners with Cape Cod Healthcare to provide a team of clinical and support staff who meet with persons hospitalized for inpatient mental health care before they leave the hospital. Our data show that 90% of patients seen by the Duffy team at Cape Cod Hospital connect to Duffy services post-discharge.

Outreach on Main Street, Hyannis 

Duffy Health Center has been a leader in developing a practical response to concerns of business owners and residents regarding individuals who had a perceived negative presence on Main Street, Hyannis. Working closely with the local police and human service agencies, Duffy teamed with a coordinated system of addressing the needs of individuals most visibly in need of services. Outreach is an effective engagement tool for encouraging people to begin the process of engaging in services that will support them in restoring their health and rebuilding their lives.
