a part of the Alternative Peer Group Model, a peer community where adolescents feel welcomed,
respected and safe as they build internal and external resources to initiate and maintain recovery
from alcohol and drug related issues. The program mission is to keep adolescents healthy,
productive, and safe by providing a peer group for young people on the road to recovery. The
Therapist will provide assessment and counseling services to the families of engaged participants
and assist in program implementation at Duffy Health Center sites at the direction of Duffy

leadership of the financial management functions for Duffy Health Center. The CFO is
responsible for overseeing the operations of Accounting, Billing, Benefits, Scheduling and
Reception. Work includes serving as a member of the leadership team, supervising staff,
developing department policies and procedures, ensuring compliance with regulations and
reporting requirements, evaluating programs, and performing administrative duties. The
CFO establishes long and short-range plans and objectives, personal performance standards
and assumes direct accountability for department results. The CFO works with senior leaders
to monitor and optimize productivity and reimbursement. This position is deemed essential*

effectively using a team-based approach. The Community Health Worker (CHW) is
responsible for examining, analyzing, and evaluating facts and circumstances surrounding
individual problems, situations, transactions and determining actions to be taken within the limits
of standard or accepted practices for patient care and scope of practice. This position is deemed

using a team-based approach. The Environment of Care Coordinator (EOCC) promotes the mission of
Duffy Health Center by providing a safe, trauma-informed, welcoming environment within Duffy
Health Center. The EOCC will use evidence-based practices (e.g., Trauma Informed Care, Diversity
Equity and Inclusion, De-Escalation techniques) to offer a compassionate and calming presence while
interacting with and assisting consumers. The EOCC will promote safety by carrying out Environment
of Care assignments, coordinating facility maintenance through multiple vendors to ensure the safety
of consumers and staff within the facility, grounds, and parking lots.

appointments with medical providers. Work includes, checking vital signs, collecting
specimens for testing, performing initial health screening/assessments, performing routine
tests, updating health and medical information, coordinating prescription call-ins to
pharmacies for doctors, scheduling appointments and providing general information and
assistance. This position is deemed essential* in the DHC Emergency Operations Plan.
Duffy Health Center is a patient-centered medical home, and every role requires functioning effectively using a team-based approach. the Patient Access Representative is responsible for providing administrative and clerical duties for Duffy Health Center. Work includes responding to requests for information and assistance from patients and providers, scheduling appointments, verifying information, processing referrals, collecting and recording data, maintaining and updating records and files, and facilitating patient care. This position is deemed essential* in the DHC Emergency Operations Plan.
Duffy Health Center is a patient-centered medical home, and every role requires functioning effectively
using a team-based approach. The Recovery Coach (RC) will provide peer recovery support services to
patients with substance use disorder (SUD) and will assist patients in achieving and sustaining recovery
as defined by each individual patient. This position will facilitate patient access to recovery-oriented
substance use disorder treatment, resources, and community recovery groups. The RC will support
patients with identifying and engaging in holistic care for their SUD across multiple systems and
frameworks. This position will maintain ongoing patient support regardless of return to use and will
serve as a motivator, ally, role model, sounding board, and advocate for patients with substance use
disorders. The RC should possess a thorough understanding of community culture to address the barriers
to successful recovery, use human experience language in place of clinical jargon, and provide links to
community resources and treatment options. This position is deemed non-essential* in the DHC
Emergency Operations Plan.
