Full Time, 40 hrs./week
 
The Recovery Coach assists pregnant and postpartum women in finding and maintaining a personalized and enhanced pathway to recovery from drug and/or alcohol substance use disorder, assists with education and access to Medication Assisted Treatment options, and supports women in enhancing skills and capacity to retain recovery/decrease relapse. The Recovery Coach assists staff/team members in the navigation of the individual’s journey. The Recovery Coach serves a unique function within the recovery continuum.
 
Demonstrates ability to maintain 2 years of sustained and uninterrupted recovery at time of hire required; 3-5 years of sustained and uninterrupted recovery preferred. Must have a Certified Addiction Recovery Coach (CARC) certification or willing to obtain and have been working in substance use disorder treatment for a minimum of 1 year, required, 2 years preferred. A candidate for this position must have a valid driver’s license and own transportation.

The Case Manager is responsible for coordinating the  care of patients with providers and outside agencies to provide support, guidance, and advocate  for the needs of Duffy Health Center clients. Work includes assessing client needs, developing a  service plan, providing interventions, advocating for clients, coordinating with community  groups, state and federal agencies, and local organizations to assist clients in obtaining  appropriate social services, resources, and assistance. Duffy Health Center is a patient-centered medical home, and every role requires functioning effectively using a team-based approach. The Post Overdose Harm Reductionist will provide support services to patients with substance use disorder (SUD) who have experienced an unintentional overdose. This position will facilitate patient access to recovery-oriented substance use disorder treatment, community resources, harm reduction resources, and provide continued community-based follow-up for at-risk patients and community members. This position is deemed non-essential* in the DHC Emergency Operations Plan.

The Recovery Coach will provide peer recovery support services to patients with substance use disorder (SUD) and will assist patients in achieving and sustaining recovery as defined by each individual patient. This position will facilitate patient access to recovery-oriented substance use disorder treatment, resources, and community recovery groups. This position is deemed non-essential* in the DHC Emergency Operations Plan. The RC will support patients with identifying and engaging in holistic care for their SUD across multiple systems and frameworks. This position will maintain ongoing patient support regardless of return to use and will serve as a motivator, ally, role model, sounding board, and advocate for patients with substance use disorders.

effectively using a team-based approach. The Young Adult Mental Health Specialist will be a
part of the Alternative Peer Group Model, a peer community where adolescents feel welcomed,
respected and safe as they build internal and external resources to initiate and maintain recovery
from alcohol and drug related issues. The program mission is to keep adolescents healthy,
productive, and safe by providing a peer group for young people on the road to recovery. The
Young Adult Mental Health Specialist will be part of identifying potential participants for the
program, acting as a resource and mentor for patients, attending group meetings, and add an
additional prospective to the APG multidisciplinary team. This position is deemed nonessential* in the DHC Emergency Operations Plan.
 
Duffy Health Center is a patient-centered medical home, and every role requires functioning effectively using a team-based approach. Position is responsible for providing clinical therapy services to clients, including individual and group treatment at Duffy Health Center’s main site and other locations at the direction of Duffy Health Center. Work includes assessing behavior, performing thorough intake assessments, diagnosing problems and developing treatment plans with measurable goals and objectives in conjunction with clinic team and outside agencies. This position is deemed essential* in the DHC Emergency Operations Plan.
 
Duffy Health Center is a patient-centered medical home, and every role requires functioning
effectively using a team-based approach. The Recovery Support Navigator is responsible for
providing systems navigation within and outside of Duffy Health Center. Patients served include
all established and establishing individuals with substance use disorders (SUD) or are on our
Medication for Addiction Treatment (MAT) program. The RSN will support in identifying
individual goals of recovery, accessing treatment services, and community resources. The
Recovery Support Navigator provides non-clinical services that engage, educate and support
individuals seeking treatment and their families in order to motivate and maintain them in health
services. Work includes assessing client needs, developing a Wellness Plan, providing
interventions, advocating for clients, coordinating with community groups, state and federal
agencies, and local organizations to assist clients in obtaining appropriate recovery services,
resources, and assistance. This position is deemed non-essential* in the DHC Emergency
Operations Plan.
 
Duffy Health center is a patient centered medical home and every role requires functioning
effectively using a team-based approach. The position will work in the SAGE general ledger,
accounts payable, payroll and grant accounting systems and modules at Duffy. The position is
one that demands some flexibility in focus and tasks as the pace of change in grant administration
can be variable. This position is deemed non-essential* in the DHC Emergency Operations Plan.
 
Duffy Health Center is a patient-centered medical home, and every role requires functioning
effectively using a team-based approach. The Clinical Risk Manager is a member of the
Management Team at Duffy Health Center. The position is responsible for monitoring
organizational systems to ensure effective health center wide quality improvement, risk
management, and patient safety. The incumbent serves as the Health Center’s Risk Manager.
This position is deemed essential* in the DHC Emergency Operations Plan.
