Full Time, 40 hrs./week
 

The Safety and Security Specialist promotes the mission of Duffy Health Center by supporting a trauma-informed, calm environment to help ensure the safety and security of clients and staff within and outside the facility. The SSS will provide a compassionate and welcoming presence primarily in the health center reception area while interacting with and assisting patients and visitors.

The position is ‘the face of Duffy” and one which requires superb people skills, judgement, and the ability to work collaboratively with multi-disciplinary team members. The SSS will independently interact and establish a positive rapport with a culturally diverse population that may be experiencing homelessness, substance use disorders and mental illness. Evidence-based training will be provided; experience with vulnerable and marginalized persons is preferred.

Full Time, 40 hrs./week

The Recovery Support Navigator is considered an essential, non-clinical member of the care team. The RSN is responsible for providing systems navigation within and outside of Duffy Health Center. Patients served include all established and establishing individuals with substance use disorders (SUD) or are on our Medication for Addiction Treatment (MAT) program. The RSN will support in identifying individual goals of recovery, accessing treatment services, and community resources.

The Recovery Support Navigator provides non-clinical services that engage, educate and support individuals seeking treatment, and their families, in order to motivate and maintain them in health services. Work includes assessing client needs, developing a Wellness Plan, providing interventions, advocating for clients, coordinating with community groups, state and federal agencies, and local organizations to assist clients in obtaining appropriate recovery services, resources, and assistance.

A candidate for this position must have a High School Diploma or GED as well as at least two (2) years of experience within substance use disorder services, preferably working with the homeless/at risk population; or an equivalent combination of education and experience.

Candidate must have lived experience with a substance abuse disorder and must identify as being in recovery. Candidate must demonstrate ability to maintain (2) years of sustained and uninterrupted recovery at time of hire required; 3-5 years of sustained and uninterrupted recovery preferred.

The Connections Case Manager is responsible for processing eligibility for insurance for patients and clients at Duffy Health Center. Work includes ensuring the accuracy and appropriateness of information received, researching, and solving problems, explaining scope of services available and providing resources and guidance where needed. Work also includes conducting patient interviews and assessing patient needs, advocating for patients, coordinating with community groups, state and federal agencies, and local organizations to assist patients in obtaining health insurance and other appropriate social services, resources, and assistance.

A candidate for this position must have a High School Diploma or GED. At least 1-2 years’ experience working with health insurance, either in a medical front desk position or billing position, is preferred. Prior case management experience is helpful but not required.

The Human Resources Director is responsible for managing and administering human resources functions for Duffy Health Center.  Work includes benefit administration, recruitment and onboarding, HR compliance, verifying credentials, interpreting and applying employment laws, employee relations and providing information and guidance to Directors and CEO regarding employment issues. 

A candidate for this position must have a bachelor’s degree in Human Resources Management, Psychology, or a related field. Five (5) or more years required in Human Resources; seven (7) or more years preferred and at least one (1) year in a medical environment. PHR or SPHR certification is preferred. Equivalent experience in absence of a degree will be considered.

The LMHT position is responsible for providing clinical therapy services to clients, including individual and group treatment at Duffy Health Center’s main site and other locations at the direction of Duffy Health Center. Work includes assessing behavior, performing thorough intake assessments, diagnosing problems and developing treatment plans with measurable goals and objectives in conjunction with clinic team and outside agencies.

A candidate for this position must have a Master’s Degree in Psychology or Social Work, and at least two (2) years of experience in social work; or an equivalent combination of education and experience.

Licenses and Certifications Required: Must be licensed as a LCSW, and/or LICSW, or LMHC and be a CPR/First Responder.

The Physician is responsible for providing primary medical care to Duffy Health Center patients at Duffy Health Center facility and outreach sites as directed by Duffy Health Center. Work includes conducting thorough examinations, issuing diagnoses and developing treatment plans, ordering tests, making referrals, authorizing prescriptions, providing follow up, and overseeing the work of medical staff.

A candidate for this position must have a Medical degree (MD or DO) and be licensed to practice medicine in the state of Massachusetts or ability to obtain license in Massachusetts, and have at least five (5) years of experience in an outpatient health clinic setting, experience in a community health center setting working with underserved populations preferred; or an equivalent combination of education and experience.

A candidate for this position must be appropriately Board Certified or Board-Eligible in Primary Care, specializing in Family Medicine, Internal Medicine, successfully complete the required credentialing process, and have a valid driver’s license.

The Director of Nursing and Clinic Operations is responsible for providing clinical therapy services to clients, including individual and group treatment at Duffy Health Center’s main site and other locations at the direction of Duffy Health Center. Collaborating with the Chief Medical Officer, the Director of Nursing and Clinic Operations develops and implements workflows, procedures, and policies, and supervises the RN and MA staff. Leads Patient-Centered Medical Home initiatives, ensures compliance with regulations and oversees care management activities.

The Director operationalizes new service line development and is seen as a leader providing health care for those experiencing homelessness in our community.

Licenses and Certifications Required: Successful candidates will have a master’s degree in nursing, active nursing license and more than five years of leadership experience in primary care operations.

Fulltime, 40 hrs./week

The Community Health Worker helps increase access to health services for people, through outreach, education, and peer support in our behavioral health department. This position provides a variety of specialized services to special populations. The purpose of this position is to refer Duffy Health Center’s patients’ who have chronic diseases to community-based agencies, the Cape Cod YMCA, and Health Living Cape Cod. The CHW is expected to understand mental illness, addiction and chronic disease being willing and ready to engage clients who struggle with these issues at all levels.

The CHW is responsible for promoting behavioral changes, assisting with linkages to appropriate health care and housing services and assisting individuals with self-management of chronic diseases. The CHW is also responsible for meeting with our partner agencies to address barriers as they arise.

Education and Licenses Required: Strong organizational, interpersonal, and social skills with the ability to collaborate with individuals from a wide range of professional and personal backgrounds. High school diploma or GED; Driver’s license required; Experience with homelessness or chronic disease encouraged.

The Alternative Peer Group RC will support adolescents, ages 13-17, enrolled within our Alternative Peer Group Program (APG.) The APG RC will work collaboratively with the team to provide direct RC support to participants enrolled in the Dennis and Falmouth programs. The APG RC will provide support to Recoverees of all gender identities through identifying and engaging in holistic care for their SUD across multiple systems and frameworks.

This position will maintain ongoing patient support regardless of return to use and will serve as a motivator, ally, role model, sounding board, and advocate for patients with substance use disorders. The Recovery Coach will additionally provide non-clinical services that engage, educate and support individuals seeking treatment and their families to motivate and maintain them in health services.

The Canal House RC will support patients placed in a community-based recovery home in Orleans. BHI, Inc. has partnered with Duffy Health Center to staff Recovery Build- An Alternative Peer Group for teens in need of recovery support. This is an innovative program that combines counseling and social activities. Recovery Build is an evidenced-based model that we plan to scale out across Cape Cod and beyond. This exciting program is a critical resource for teens.

We are seeking an energetic, youth-focused licensed therapist who is a self-starter to join us in promoting, developing and maintaining this project. The position is 32 hours per week and will expand as our fundraising efforts support. Candidate must love working with adolescents, is knowledgeable about addiction and youth recovery and works well with youth and their families.

Full Time, 40 hrs./week

The accountant is responsible for assisting in the management and the administration of accounting functions including acting as a backup, if needed, to the accountant staff and, if needed, assisting the Chief Financial Officer (CFO). As needed, the position will work in the SAGE general ledger, accounts payable, payroll and grant accounting systems and modules at Duffy. The position is one that demands some flexibility in focus and tasks as the pace of change in grant administration can be somewhat unpredictable. 

At minimum, an Associate Degree in Accounting. BS/BA in Accounting is preferred. Experience of at least five (5) years in general ledger/grant accounting in a community health center or in a health care provider will be seriously considered in lieu of more formal advanced training, as will accounting experience in a similar not-for-profit environment.
