The Peer Mentor will be a part of the Alternative Peer Group Model, a peer community where adolescents feel welcomed, respected and safe as they build internal and external resources to initiate and maintain recovery from alcohol and drug related issues.  The program mission is to keep adolescents healthy, productive, and safe by providing a peer group for young people on the road to recovery.  The Peer Mentor will be part of identifying potential participants for the program, acting as a resource and mentor for patients, attending group meetings, and add an additional prospective to the APG multidisciplinary team. 

A candidate for this position must have a minimum of a High School Diploma or educational equivalent.  Candidate must have maintained 2 years of sustained and uninterrupted recovery at time of hire; 3-5 years of sustained and uninterrupted recovery preferred.  A valid driver’s license is required.

Full time, 40 hrs./week

The Health Information Technology (HIT) Coordinator is a crucial member of the team managing the Duffy Health Center electronic health record for patients.  The HIT Coordinator should be highly proficient with the electronic health record software and have a good clinical background and an aptitude for computer systems.  The HIT Coordinator works across all departments to develop training materials and to conduct training sessions.  They assist in configuring, implementing, maintaining, and supporting the electronic health record. Additionally, recommendations for procedural or process changes to ensure the effectiveness and efficiency of the system, taking into consideration company and/or departmental policies and procedures will be expected of the position.  This position delivers “at-the-elbow” support as needed. 

A candidate for this position should have at least a high school diploma, or GED, and have at least three (3) years of experience working within a clinical setting.  Five (5) years experience working in an electronic health record is preferred.

The Telehealth Navigator will be responsible for working with clients to guide them through accessing telehealth services through Duffy Health Center.  The Telehealth Navigator will work with staff in order to best serve the clients’ telehealth needs by receiving referrals and acting upon them in a timely manner. 

A candidate for this position should have at least a high school diploma, or GED.  An Associates Degree in a computer related field of study or equivalent experience.  Two (2) or more years of IT experience related to customer support, training, and /or education, ideally in a clinical setting is preferred.

Part time, 20 hrs./week

The Recovery Coach will provide peer recovery support services to patients with substance use disorder (SUD) and will assist patients in achieving and sustaining recovery as defined by each individual patient. This position will facilitate patient access to recovery-oriented substance use disorder treatment, resources, and community recovery groups.  They will also support patients with identifying and engaging in holistic care for their SUD across multiple systems and frameworks. This position will maintain ongoing patient support regardless of return to use and will serve as a motivator, ally, role model, sounding board, and advocate for patients with substance use disorders.

A candidate for this position must have at least a High School diploma or GED, demonstrates ability to maintain 2 years of sustained and uninterrupted recovery at time of hire; 3-5 years of sustained and uninterrupted recovery preferred.  A candidate must have a Certified Addiction Recovery Coach (CARC) certification and have been working in substance use disorder treatment for a minimum of 1 year, required, 2 years preferred.

The Licensed Practical Nurse (LPN) is responsible for observing patients, charting and reporting changes in conditions, measuring and recording patient vital signs and providing basic patient care or treatment such as medication administration.  The LPN works within an integrated health care team collaborating with and supporting the Psychiatric Providers. The LPN will have effective and therapeutic communication skills while demonstrating compassion through positive relationship development.

A candidate for this position must have an Associate’s Degree, Bachelor’s preferred, valid LPN license and at least one (1) year of experience, preferably working with the homeless/at risk population; or an equivalent combination of education and experience.

The IT Special Projects Coordinator is responsible for providing IT support for the staff of Duffy Health Center and is a crucial member of the Organizational Development team. This position acts as a project lead for new software initiatives and for on-going support of implemented applications.

A candidate for this position should have at least an associate degree in Information Technology, and one (1) to two (2) years of experience in project management/coordination or one (1) to two (2) years implementation and/or business analysis experience. The Moms Do Care program supports pregnant and parenting women who are seeking recovery from a history of substance use disorders.  The Licensed Mental Health Therapist will provide assessment and counseling services, group-based therapy.  The Therapist will also assist in program implementation and development at Duffy Health Center sites at the direction of Duffy Health Center.

A candidate for this position must have a Master’s Degree in Psychology or Social Work, and at least two (2) years of experience in social work; or an equivalent combination of education and experience. Must be licensed as a LCSW, and/or LICSW, or LMHC, and be CPR/BLS certified.

The position is responsible for providing systems navigation within and outside of Duffy Health Center. Patients served include all established and establishing individuals with substance use disorders (SUD) or are on our Medication for Addiction Treatment (MAT) program. The RSN will support in identifying individual goals of recovery, accessing treatment services, and community resources. The Recovery Support Navigator provides non-clinical services that engage, educate and support individuals seeking treatment, and their families, in order to motivate and maintain them in health services. Work includes assessing client needs, developing a Wellness Plan, providing interventions, advocating for clients, coordinating with community groups, state and federal agencies, and local organizations to assist clients in obtaining appropriate recovery services, resources, and assistance.

A candidate for this position must have a High School Diploma or GED as well as at least two (2) years of experience within substance use disorder services, preferably working with the homeless/at risk population; or an equivalent combination of education and experience.  

In addition, a candidate must have lived experience with addiction and must identify as being in recovery. Candidate must demonstrate ability to maintain (2) years of sustained and uninterrupted recovery at time of hire required; 3-5 years of sustained and uninterrupted recovery preferred.

The Connections Case Manager is responsible for performing various activities that facilitate a patient’s access to insurance coverage. Position is responsible for coordinating with patients, providers, and outside agencies to provide support, guidance, and advocate for the needs of Duffy Health Center patients. Work includes conducting patient interviews and assessing patient needs, advocating for patients, coordinating with community groups, state and federal agencies, and local organizations to assist patients in obtaining health insurance and other appropriate social services, resources, and assistance. The position is responsible for gathering, reviewing, entering and scanning all patient data including demographic information, insurance information and patient consents.

A candidate for this position must have a basic knowledge of: state and federally sponsored insurance programs including MassHealth, Medicare and Health Safety Net; issues related to homeless and at-risk individuals, including mental illness, substance abuse, criminal history, and physical and sexual abuse; protocol for handling crisis situations; Title VI of the Civil Rights Act of 1964, including National Origin Discrimination; HIPAA regulations.

The Physician is responsible for providing primary medical care to Duffy Health Center patients at Duffy Health Center facility and outreach sites as directed by Duffy Health Center. Work includes conducting thorough examinations, issuing diagnoses and developing treatment plans, ordering tests, making referrals, authorizing prescriptions, providing follow up, and overseeing the work of medical staff.

A candidate for this position must have a Medical degree (MD or DO) and be licensed to practice medicine in the state of Massachusetts or ability to obtain license in Massachusetts, and have at least five (5) years of experience in an outpatient health clinic setting, experience in a community health center setting working with underserved populations preferred; or an equivalent combination of education and experience.

A candidate for this position must be appropriately Board Certified or Board-Eligible in Primary Care, specializing in Family Medicine, Internal Medicine, successfully complete the required credentialing process, and have a valid driver’s license.
