The LMHT position is responsible for providing clinical therapy services to clients, including individual and group treatment at Duffy Health Center’s main site and other locations at the direction of Duffy Health Center. Work includes assessing behavior, performing thorough intake assessments, diagnosing problems and developing treatment plans with measurable goals and objectives in conjunction with clinic team and outside agencies.

A candidate for this position must have a Master’s Degree in Psychology or Social Work, and at least two (2) years of experience in social work; or an equivalent combination of education and experience.

The Lead Patient Access Representative (PAR) is responsible for providing guidance and peer support in the daily workflow of the Patient Access Representative team. The Lead PAR performs administrative and clerical duties for Duffy Health Center. Work includes responding to requests for information and assistance from patients and providers, scheduling appointments, verifying information, processing referrals, collecting and recording data, maintaining and updating records and files, and facilitating patient care.

The Young Adult Mental Health Specialist will be a part of the Alternative Peer Group Model, a peer community where adolescents feel welcomed, respected, and safe as they build internal and external resources to initiate and maintain recovery from alcohol and drug related issues. The program mission is to keep adolescents healthy, productive, and safe by providing a peer group for young people on the road to recovery.

The Young Adult Mental Health Specialist will be part of identifying potential participants for the program, acting as a resource and mentor for patients, attending group meetings, and add an additional prospective to the APG multidisciplinary team. This position is deemed non-essential* in the DHC Emergency Operations Plan.

A candidate for this position must have a minimum of a High School Diploma or educational equivalent, bachelor’s degree preferred. Lived experience with substance use disorder preferred. For applicants with lived experience with substance use disorder, 2 years of sustained and uninterrupted recovery at time of hire; 3-5 years of sustained and uninterrupted recovery preferred. The position is responsible for providing systems navigation within and outside of Duffy Health Center. Patients served include all established and establishing individuals with substance use disorders (SUD) or are on our Medication for Addiction Treatment (MAT) program. The RSN will support in identifying individual goals of recovery, accessing treatment services, and community resources. The Recovery Support Navigator provides non-clinical services that engage, educate and support individuals seeking treatment, and their families, in order to motivate and maintain them in health services. Work includes assessing client needs, developing a Wellness Plan, providing interventions, advocating for clients, coordinating with community groups, state and federal agencies, and local organizations to assist clients in obtaining appropriate recovery services, resources, and assistance.

A candidate for this position must have a High School Diploma or GED as well as at least two (2) years of experience within substance use disorder services, preferably working with the homeless/at risk population; or an equivalent combination of education and experience.  

In addition, a candidate must have lived experience with addiction and must identify as being in recovery. Candidate must demonstrate ability to maintain (2) years of sustained and uninterrupted recovery at time of hire required; 3-5 years of sustained and uninterrupted recovery preferred.

Work includes conducting thorough examinations, issuing diagnoses and developing treatment plans, ordering tests, making referrals, authorizing prescriptions, providing follow up, and overseeing the work of medical staff.

A candidate for this position must have a Medical degree (MD or DO) and be licensed to practice medicine in the state of Massachusetts or ability to obtain license in Massachusetts, and have at least five (5) years of experience in an outpatient health clinic setting, experience in a community health center setting working with underserved populations preferred; or an equivalent combination of education and experience.

The Director of Clinic Operations is responsible for providing clinical therapy services to clients, including individual and group treatment at Duffy Health Center’s main site and other locations at the direction of Duffy Health Center. Collaborating with the Chief Medical Officer, the Director of Clinic Operations develops and implements workflows, procedures, and policies, and supervises the RN and MA staff. Leads Patient-Centered Medical Home initiatives, ensures compliance with regulations and oversees care management activities.

The Director operationalizes new service line development and is seen as a leader providing health care for those experiencing homelessness in our community.

Licenses and Certifications Required: Successful candidates will have a master’s degree in nursing, active nursing license and more than five years of leadership experience in primary care operations.

The Administrative Assistant is responsible for providing a variety of administrative and clerical duties for Duffy Health Center, including collecting and recording data, maintaining, and updating records and files for department meetings, and minute taking. The Community Health Worker helps increase access to health services for people, through outreach, education, and peer support in our behavioral health department. This position provides a variety of specialized services to special populations. The purpose of this position is to refer Duffy Health Center’s patients’ who have chronic diseases to community-based agencies, the Cape Cod YMCA, and Health Living Cape Cod. The CHW is expected to understand mental illness, addiction and chronic disease being willing and ready to engage clients who struggle with these issues at all levels.

The CHW is responsible for promoting behavioral changes, assisting with linkages to appropriate health care and housing services and assisting individuals with self-management of chronic diseases. The CHW is also responsible for meeting with our partner agencies to address barriers as they arise.

Education and Licenses Required: Strong organizational, interpersonal, and social skills with the ability to collaborate with individuals from a wide range of professional and personal backgrounds. High school diploma or GED; Driver’s license required; Experience with homelessness or chronic disease encouraged.

The Alternative Peer Group RC will support adolescents, ages 13-17, enrolled within our Alternative Peer Group Program (APG.) The APG RC will work collaboratively with the team to provide direct RC support to participants enrolled in the Dennis and Falmouth programs. The APG RC will provide support to Recoverees of all gender identities through identifying and engaging in holistic care for their SUD across multiple systems and frameworks.

This position will maintain ongoing patient support regardless of return to use and will serve as a motivator, ally, role model, sounding board, and advocate for patients with substance use disorders. The Recovery Coach will additionally provide non-clinical services that engage, educate and support individuals seeking treatment and their families to motivate and maintain them in health services.

The Canal House RC will support patients placed in a community-based recovery home in Orleans. BHI, Inc. has partnered with Duffy Health Center to staff Recovery Build- An Alternative Peer Group for teens in need of recovery support. This is an innovative program that combines counseling and social activities. Recovery Build is an evidenced-based model that we plan to scale out across Cape Cod and beyond. This exciting program is a critical resource for teens.

We are seeking an energetic, youth-focused licensed therapist who is a self-starter to join us in promoting, developing and maintaining this project. The position is 32 hours per week and will expand as our fundraising efforts support. Candidate must love working with adolescents, is knowledgeable about addiction and youth recovery and works well with youth and their families.
