Our mental health team helps individuals address their underlying mental health conditions so that they are able to live healthy, productive lives. Our mental health services are an important part of our integrated care model. 

Mental Health Programs

All Duffy patients are eligible for individual therapy. Therapists are part of a patient’s care team and are in communication with their primary care provider, case manager, and other members of the team, all working together to support the patient in achieving their goals. 

Therapy sessions are provided regularly, with the schedule depending on the patient’s needs.

Customized support services are available for LGBTQIA+ patients.

Duffy therapists are an important part of each patient’s care team. There is a therapist embedded on the Office Based Addictions Team (OBAT) who specializes in substance use disorders and is available to individuals receiving OBAT services. There is also a therapist located on the medical floor for individuals’ needs.

Our psychiatric team includes a Psychiatrist and three Psychiatric Nurse Practitioners who are available to assist in supporting patients’ mental health needs. 

With funding from our federal Health Care for the Homeless grant, Duffy partners with Cape Cod Healthcare to provide a team of clinical and support staff who meet with persons hospitalized for inpatient mental health care before they leave the hospital. Our data show that 90% of patients seen by the Duffy team at Cape Cod Hospital connect to Duffy services post-discharge.

The Pathways to Hope team provides meaningful and compassionate connection to individuals who have expressed suicidal ideation, while also working closely with the care team to ensure adherence to a safety plan. 

A therapist from Duffy Health Center has regular hours at the Barnstable County Correctional Facility in order to provide support to inmates as they transition into the community post-release.

Duffy therapists are regularly available to provide court-ordered evaluations.
