Our mission is to provide equitable, integrated health care and support services to persons who are experiencing homelessness or are at risk of homelessness on Cape Cod, and to improve the quality of life for vulnerable and marginalized populations through community collaborations, leadership, and advocacy. We envision Cape Cod as a community where everyone has access to quality health care, safe affordable housing, and a life filled with hope and purpose.

Legacy Gifts*
- Consider leaving a lasting legacy to Duffy Health Center*. We invite you to consider one or more of the following options:
- Leave a planned gift in your will or a revocable trust.
- Name Duffy Health Center as a beneficiary of a retirement account, IRAs, 401(K)s, and other qualified plans.
- Name Duffy Health Center as a beneficiary of your life insurance policy or annuity.
