Our mission is to provide equitable, integrated health care and support services to persons who are experiencing homelessness or are at risk of homelessness on Cape Cod, and to improve the quality of life for vulnerable and marginalized populations through community collaborations, leadership, and advocacy. We envision Cape Cod as a community where everyone has access to quality health care, safe affordable housing, and a life filled with hope and purpose. 

 Every donation given by those who support our work is important and meaningful. All donations made to Duffy directly support our many programs and services. If you would like, you can choose which program or fund that your gift supports. Our programs are listed below.

Legacy Gifts*

  1. Consider leaving a lasting legacy to Duffy Health Center*. We invite you to consider one or more of the following options:
    1. Leave a planned gift in your will or a revocable trust.
    2. Name Duffy Health Center as a beneficiary of a retirement account, IRAs, 401(K)s, and other qualified plans.
    3. Name Duffy Health Center as a beneficiary of your life insurance policy or annuity.

Our General Operating Fund keeps the Duffy mission alive by supporting our passionate and dedicated staff, and sustains all of our programs and services for those experiencing homelessness or at-risk of homelessness across Cape Cod. 

This fund is used to provide ongoing trainings; seed money for innovative new programs; and covers costs that are not reimbursed by government programs. Our General Operating Fund is vital. Without it we wouldn’t be able to make the difference we do.

Housing First Fund

Provide the comforts of home. 

The Housing First model is nationally-recognized and is one of our most active funds. We support our clients as they move into stable housing, an underpinning of overall physical and mental health outcomes. Many clients need basic necessities as they transition into new housing such as beds, dressers, tables, and chairs. The Housing First Fund also helps with rent requirements such as first and last month’s rent or deposits to secure client housing. These are often steep costs for our clients but are an absolute necessity to gaining access to stable housing.

In From the Streets

Health is housing.

For over 15 years, In From the Streets has made a difference to hundreds of Cape Codders who need emergency shelter due to severe weather or medical frailty, keeping people safe from the elements and other risks. This is a multi-faceted, regional program that Duffy administers on behalf of human service agencies across Barnstable County. The program includes a provision of short-term motel shelter and, importantly, affords Duffy case managers time with clients to coordinate more permanent housing options, to avail themselves of community resources, and to stay in treatment and care.

“Not only did I need surgery, but the doctor said I would also need a week of bedrest to recover. I was homeless at that time - ‘bedrest’ was not an option. Luckily my doctor at Duffy was able to get me a place to stay for the week so that I could recover after my surgery and be on a better path to health.” - Duffy Client

Bridge Fund

Supporting patients and clients on their road to recovery. 

The Bridge Fund provides support for patients and clients as they transition into housing that supports their recovery needs. Safe, stable housing is instrumental to successful recovery but there are often many financial barriers that prevent our patients from accessing it. This fund supports first and last months’ rents, as well as security deposits, and provides necessities for clients’ new housing.

“I was living in a bad place. I was so excited when I had the chance to get into sober housing, but then I saw the cost and knew I couldn’t afford it. My Recovery Support Navigator at Duffy told me they might be able to help... and they did. If it weren’t for them, I wouldn’t have had the opportunity to work on my recovery.” - Duffy Client

Small Favors Fund

Help with the little - but necessary - things.

Our staff often receive client requests for small necessities like a haircut for a job interview, food gift cards to get by for the weekend, toiletries, a prescription copay, and other small favors. This Fund was created to support these important needs for our clients and patients. With it, our case managers can respond to small problems swiftly, avoiding bigger programs down the road.

My case manager helps me with everything from housing applications to getting a pair of clean socks and a toothbrush. Building Fund

Improve our infrastructure, clinical space and technology.

Gifts to our Building Fund are used in support of Duffy’s mission and vision in tangible ways. As the need for our programs and services evolves, so does the need to advance our infrastructure. This includes technology, exam rooms, shared spaces such as conference rooms and new provider workspaces. Duffy staff, patients and clients benefit directly from your generosity. Contributions to our Building Fund enable us to advance our care and support of our patients and clients every day.

Jeanne Marie Shriber Fund

Provide continuing education for staff.

Established in December 2012, this fund was created in memory of Jeanne by her sister, Judy Best-Lavigniac, and the Duffy family. Jeanne worked for many years as a nurse and understood the importance of ongoing training and education for nurses to move forward in their careers. In her honor, the family has requested that funds be used to help provide Duffy staff with opportunities to further their education and training. 

"As a Nurse Practitioner, I know firsthand how important the work is that Medical Assistants and other entry level providers do. Creating a career path so that staff can develop new skills while remaining on the job is a win-win opportunity for everyone. Thank you for helping create the Jeanne Marie Shriber Fund. My sister would be honored to know what a difference she is making."

Community of Hope

One of the most impactful ways to give is to become a member of our Community of Hope monthly giving program. Monthly giving is especially meaningful for Duffy because it provides a steady stream of revenue on which we can depend, and also provides supporters with an opportunity to make an ongoing contribution over time. No investment is too small; you can join our Community of Hope with as little as $10 or $20 a month.
