Our services include medical care, mental health & substance use disorder programs, case management, health insurance & benefits, and spiritual care. Talk to your provider or case manager if you have questions about a particular service.
Please call (508) 771-9599 and make an appointment with our Connections Case Management team for any questions you have. They can also help assess your insurance eligibility.
Duffy Health Center maintains an electronic medical record for each of our patients. The record contains a history of the care provided by Duffy Health Center. For some patients, their record will contain information transferred to us by other providers. Your medical records are confidential and are the property of Duffy Health Center. You have the right to request a copy of your medical record. Medical records are released according to the Duffy Health Center Record Release policy.
