Improving your mental and emotional well-being 

Many Duffy patients have chronic disabilities, including mental health disorders. Many suffer from trauma such as domestic violence, childhood abuse, prior history of incarceration and unstable housing situations.

We help individuals address their underlying mental health conditions so that they are able to care for their chronic medical conditions. Our mental health services are an important part of our integrated care model.

Services include:

  • Screening and assessment

  • Psychiatric services

  • Individual and group counseling

  • Support services for gay, lesbian, bisexual and transgender people

  • Medication management

  • Court evaluations

  • In-reach to Barnstable County Correctional Facility

  • Care coordination with Cape Cod Healthcare and other facilities
