REACH: Respectful, Equitable Access to Compassionate Healthcare
Duffy Health Center is bringing care on the road with our mobile health and outreach unit. The goal of the REACH unit is to provide access to important services to members of our community for whom transportation may be a barrier.
The unit is staffed by an Outreach Case Manager and a Post-Overdose Harm Reductionist. The team is out in our community three days each week in the communities of Yarmouth, Dennis, Falmouth, Mashpee, and Hyannis.

Services include:
- Access to showers and basic hygiene
- Recovery support navigation
- Access to medication for opioid use disorder through telehealth with Duffy providers
- Case management resources
- Harm reduction resources including Narcan and test strips
These services are available to individuals in need as well as loved ones looking for support.
