Primary care is the range of health services that includes preventative medicine, treatment for common illnesses, and referrals to specialists; anything related to helping you achieve your best health. 

Duffy Health Center takes a personal approach to health, offering in-person visits, telehealth appointments, shared medical appointments, and care coordination. Each patient has a care team, a group of professionals working alongside them to support them in restoring their health and rebuilding their lives.

Medical Services

  • Chronic disease management
  • Referrals to external medical specialists
  • Referrals to internal support services
  • Identification and treatment of infections such as Hepatitis C and syphilis 
  • Women’s health services, including gynecology
  • Initiation and management of gender-affirming hormone therapy
  • Support managing hypertension (high blood pressure) at home
  • Education and support managing diabetes
  • Prevention and treatment of Hepatitis C
  • Initiation and management of HIV treatment 

Duffy Health Center has an established relationship with Cape Cod Healthcare. We can provide:

  • Assistance managing care after a stay at the hospital
  • Assistance managing care after a visit to the Emergency Department
  • Internal referrals to Duffy Health Center case managers for support with housing, SNAP benefits, and other resources
  • Internal referrals to Duffy Health Center therapists and spiritual care provider
  • Visits with a credentialed nutritionist for education and counseling
  • Assistance managing diet for better diabetes and blood pressure outcomes

Duffy Health Center has a Psychiatrist and Psychiatric Nurse Practitioners on our team to support patients in managing their mental health along with their physical health.
