Chief Executive Officer
Heidi Nelson joined Duffy Health Center as CEO in 2010. Heidi is responsible for Duffy Health Center operations, quality of care, program development, funding support, advocacy on behalf of persons experiencing homelessness, and representing Duffy at the local, state and national level. Previously, Heidi was the Executive Director of Heartland Health Outreach, Inc. Prior to that, she was Assistant Vice President at Northwestern Memorial Hospital in Chicago.Heidi is the Administrators Committee Chair for the National Health Care for the Homeless Council. She has a bachelor’s degree in Community Health Education from the University of Illinois and a master’s degree in health services administration from the University of Michigan School of Public Health. Heidi also completed a Fellowship in Academic Medical Center Administration at the University of Michigan Medical Center, and she is also Fellow of the American College of Healthcare Executives.
Heidi lives in Harwich with her husband.
Chief Financial Officer
Jeff is a Selectman in the Town of Chatham where he lives with his wife. He is also on the Board of Directors at the Cape Cod Community College Educational Foundation. He has previously served on the Board of the Cape and Islands United Way and as President of the Massachusetts-Rhode Island Chapter of the Healthcare Financial Management Association.Senior Director of Program Resources
Jane de Groot joined Duffy In 2006 as the Director of Program Resources. Jane has initiated and nurtured Duffy’s involvement in research with academic partners; has championed evidence-based practice training for staff and community partners; and has strengthened Duffy’s ability to serve its clients through developing key agency partnerships and special projects.Jane received her bachelor’s degree in Comparative Literature from Antioch College and MPA in Health Services Administration from NYU Wagner School of Public Service. In New York, she worked for John Wiley & Sons Medical Division, Memorial Sloan-Kettering Cancer Center, the New York City Dept. of Health, and NYU School of Education and Allied Health, Office of Sponsored Research.
Chief Development and Community Engagement Officer
Ms. Grambach has lived and worked on Cape Cod for more than ten years in development, marketing and communications roles at Cape Cod Healthcare and other area non-profits. She holds a BA in Journalism from The University of Georgia and an MS in Health Communications from Tufts University School of Medicine. Ms. Grambach began her career working in health communications and health care operations with Genzyme Corporation and Partners HealthCare.Sara has served as a Town Councilor for the Town of Barnstable and on the board of Cape Cod Young Professionals.
In her role at Duffy, Ms. Grambach oversees all aspects of fundraising and the center’s marketing and communications efforts.
Chief Medical Officer
Dr. Jones came to Duffy from the role of Chief Quality and Compliance Officer at Greater New Bedford Community Health Center. She obtained a Bachelor of Arts degree in Biology and a Master’s Degree in Education at the University of Michigan before completing her Doctorate of Medicine at Michigan State University College of Human Medicine. She is pursuing her Master’s of Business Administration at UMass Amherst.Dr. Jones completed her Obstetrics and Gynecology Residency at Oakwood Hospital and Medical Center in Dearborn, Michigan and is a Certified Physician Executive through the American Association for Physician Leadership. In her role at Duffy, she oversees the center’s medical department including patient care, patient safety, quality improvement, and clinical operations.
Director of Compliance and Operations Improvement
Anna has been at Duffy since January 2018 first as Associate Director of Quality Improvement and now as Director of Compliance and Operations Improvement. She has a BS in Biology and Chemistry from Boston College, Master of Health Administration from Duke University and a Master of Public Policy from UMass Dartmouth. As Director of Compliance and Operations Improvement, she is responsible for compliance, risk management coordination, operations improvement, emergency planning and serves as safety and compliance officer. Prior to working at Duffy she worked in various senior operations management roles at several Boston teaching hospitals and at the Public Policy Center at UMass Dartmouth. Anna serves as a Trustee for the Beth Israel Deaconess Hospital – Plymouth and is a member of the Diocesan Pastoral Council in the Diocese of Fall River. She resides in Sagamore Beach.Director of Substance Use Disorder and Outreach Programs
Since joining Duffy in 2019, Daniel Rodrigues has supported the agency in strengthening interventions and initiatives geared towards improving access to treatment for individuals experiencing substance use and mental health challenges. Daniel is an active participant in multiple coalitions on Cape Cod and is a co-chair of the Barnstable County Regional Substance Addiction Council. Daniel received his bachelor’s degree in Psychology and Portuguese from the University of Massachusetts Amherst before completing his Master’s Degree in Social Work from Smith College School for Social Work. Daniel is a Licensed Independent Clinical Social Worker and holds a Perinatal Mental Health Certificate. Prior to joining Duffy, Daniel’s work experience included inpatient, outpatient, and residential treatment for co-occurring disorders, as well as hospital based social work serving families admitted to Maternity and Pediatrics.Director of Behavioral Health and Care Management
Erin Russell joined Duffy Health Center in 2022 as Director of Behavioral Health. Since joining the team at Duffy, she has worked to provide integrated behavioral health services and improve access to the behavioral health department. Erin has worked as a Master's level social worker since 2012 with experience in trauma informed care, co-occurring diagnosis, leadership, clinical supervision, case management, group programming and individual counseling. Erin earned a Bachelor of Science in Psychology from Bridgewater State University and a Master of Social Work from Boston University and currently is a Licensed Independent Social Worker. Erin currently works with numerous other social services community agencies to enhance behavioral health and wellness for the Cape community and is a co-chair of the Mass Leagues Behavioral Health Directors meetings.Board of Directors

Consumer Advisory Board
Representing Community Voices
The Consumer Advisory Board (CAB) is an appointed group of Consumers whose input is sought for strategic and operations improvement purposes on an ongoing basis. The primary mission of the CAB is to facilitate open communication between Duffy Health Center leadership and Consumers.
The CAB is a crucial source of insights about Consumer issues and concerns and service requirements from Duffy. Members have the opportunity to influence decision-making at the highest levels of Duffy and to experience meaningful interaction with peers.
