Programs

New CHANCE Program

New CHANCE targets persons who have lived in long-term homelessness with disabilities. The goal is to house these individuals and help them to access services and benefits. New CHANCE also works at the community level through the community consortium and at the state level through a policy-focused steering committee to make lasting change.

Office-Based Opioid Treatment (OBOT) Program

The OBOT Program offers medical treatment with mental health therapy to address both the physical and psychological effects of opioid addiction. Our OBOT medical and behavioral health team works closely with many agencies, including the Barnstable County Dept. of Probation, to help persons reach their goals for recovery, including housing, employment, and risk reduction. In addition, Duffy has been assisting other community health centers to replicate this successful program within their own facilities.

Opportunities Program

The Opportunities Program provides intensive case management for homeless persons with substance abuse issues. Veterans are a special focus and Duffy works closely with local veterans’ agencies to bridge the service gap, attain housing, and to ensure veterans receive all their entitlements. The intensive case management encourages engagement and trust; this five year program has documented increases in housing, employment, and decreases in utilization of public services (e.g., emergency room visits, involvement with the criminal justice system, detox and shelter stays).
